The Carleton Place & District Memorial Hospital Foundation (Foundation) is committed to protecting your privacy. We value your trust and recognize that maintaining this trust requires that we be open and accountable in our treatment of the Personal Information you share with us. We do not trade, rent or sell the Personal Information of our valued supporters.
The following principles reflect our pledge to safeguard Personal Information pursuant to this Privacy Statement.
2. We are accountable for Personal Information.
"Personal Information" is any information that can be used to identify a specific individual. This information includes whether an individual has donated to the Foundation, how much has been donated and an individual's private contact information. This information can further include an individual's opinions or beliefs, as well as facts about, or related to, the individual. Business contact information and certain publicly available information, such as names, addresses and telephone numbers that are published in telephone directories, are not considered personal information.
We keep the Personal Information that is provided to us in strict confidence.
Our employees, Board Members and those acting on behalf of the Foundation for fundraising purposes, sign confidentiality agreements. Safeguards are in place to ensure that the information is not accessed, disclosed or shared more widely than is necessary to achieve the purpose for which it was gathered. We also take measures to ensure the integrity of this information is maintained and to prevent its being lost or destroyed. You may access your Personal Information at any time to ensure that it is correct and current.
3. We safeguard Personal Information.
Personal Information is protected with security safeguards appropriate to the sensitivity of the information. We use password protocols and secure websites to protect the Personal Information we receive when an online donation is made. Our software is routinely updated to maximize protection of such information.
4. We collect, use and disclose Personal Information for the purposes that we identify to you.
We may collect information from you in the following ways:
- Information that is provided to us by you;
- Information we collect about you from our interactions with you (for example, when using our website or other technology);
- Information we obtain from the Hospital in accordance with their Privacy of Personal Information Policy IX-28; and
- Information we obtain from third-party sources (such as in the phone directory or public mailing list).
When the Foundation collects Personal Information, it is for the following purposes:
- fundraising;
- the administration of donations;
- providing information about the activities of the Foundation and Hospital;
- for the purpose of evaluating and improving the Foundation and its activities;
- to comply with legal and regulatory requirements; and
- to ask for support in our mission to strengthen the Hospital’s ability to provide high quality health care for patients in communities we serve.
Cookies and Passive Collection (Tracking Technologies)
The Foundation website uses “cookies” to help you personalize your online experience. A cookie is a text file that is placed in your web browser’s memory by a web server. Cookies cannot be used to run programs or to deliver malicious software to your computer. One of the primary purposes of cookies is for convenience and to save you time. The purpose of a cookie is to tell the web server that you have returned to a specific page. For example, if you choose to make a donation through our online website services, a cookie helps the site recall your specific information on subsequent visits. This simplifies the process of recording your personal information, such as your name and billing addresses. When you return to the website on another occasion, the information you previously provided can be retrieved, so you can easily use the features that you customized. Accepting or declining cookies is up to you. Most web browsers (including Firefox, Google Chrome, Internet Explorer, Edge, Safari, and Opera) are usually set to accept cookies by default, but you can easily modify your settings to decline them if you prefer. If you choose to decline cookies, you may not be able to fully experience the interactive features of our website or other sites you may visit.
5. We obtain consent to collect, use or disclose Personal Information.
We ONLY collect, use or disclose Personal Information with your permission. Permission may be expressed orally, in writing or may be implied, and it may be provided to us or a third party orally, electronically or in writing. You may limit or opt-out of future contact by the Foundation by contacting us at foundation@carletonplacehosp.com.
6. We limit the collection of Personal Information
We limit the collection of Personal Information to that which is necessary for the purposes identified. Personal Information will be collected only by fair and lawful means.
7. We limit the use, disclosure and retention of Personal Information
Personal Information will not be used or disclosed for purposes other than those for which it was collected, except with consent or as required by law. Personal Information will be retained only as long as necessary for the fulfillment of those purposes. The Foundation does not trade, rent or sell any Personal Information to third parties.
Your Personal Information will be used by committee members and volunteers acting on behalf of the Foundation when fundraising, stewarding and thanking for gifts: basic donation information is limited and only shared on an as needed basis that may include the range of cumulative gifts (as would be public on the donor wall), length of donation history and the date of your last gift.
We only disclose information to the following persons:
- other registered charities when we conduct joint fundraising activities with them;
- law enforcement or Canada Revenue Agency officials if it relates to a criminal investigation, alleged illegal activity, or as required by law; or
- other persons, with your consent.
We also provide or permit access to Personal Information to certain service providers to the extent they need the information to conduct activities on our behalf such as to process donations as described below.
Where we choose to have certain service providers conduct activities on our behalf (such as some fundraising and processing services), we select the service providers very carefully and take all reasonable precautions to ensure that the service provider protects your Personal Information. Some of our service providers are located outside of Canada. As a result, unless there is a legal or regulatory requirement to keep such information in Canada, your Personal Information may be stored or processed outside of Canada and your Personal Information may be accessible to law enforcement and/or regulatory authorities in accordance with the laws of these jurisdictions.
For example, the Foundation’s web pages contain online forms that allow visitors to make a donation. The personal and credit card information provided on these forms is used only to process these donations. Online donations to the Foundation may be processed through third parties. The security and privacy policies of these third parties are available at the bottom of the web page and by clicking on the "Verified and Secured" link on the online donation form web pages.
8. We allow you to access your Personal Information.
You may ask us whether we hold any Personal Information about you. 9. Accuracy of Personal Information.
We rely on you to provide accurate Personal Information. 10. Compliance with our privacy practices.
If there is concern about the Foundation's compliance with our Privacy Policy, please contact the Executive Director at Carleton Place & District Memorial Hospital Foundation, 211 Lake Avenue East, Carleton Place, K7C 1J4 or by sending an email to foundation@carletonplacehosp.com. We will investigate all complaints. If a complaint identifies a gap in compliance, we will take appropriate steps to remedy the situation, including changing our policies and practices if necessary.
